Association of Insight Level with False-Negative Alcoholism Screening Tests in Alcohol-Dependent Patients

Abstract
|
|
|
Objectives£ºWe investigated the association of insight level with false-negative alcoholism screening tests in alcoholdependent patients.
Methods£ºThe subjects were 89 patients (male 81, female 8) who met DSM-IV criteria for alcohol dependence and were admitted to a psychiatric hospital in Korea. All subjects participated in a 3-month inpatient program of 10-30 hours duration per week, consisting of Alcoholics Anonymous (AA), group therapy, cognitive-behavioral therapy, and other sessions for alcohol dependence. All subjects completed the National Alcoholism Screening Test (NAST) and the Hanil Alcohol Insight Scale (HAIS) twice, just after admission and just before discharge. The results of the NAST were compared among three groups divided by insight level (poor, partial, and good).
Results£º1) The subject was 43.7¡¾10.9 (mean¡¾ S.D.) years old. Among the subjects, 58.4% were married, 40.4% did not have any religion, and 38.4% were unemployed. Seventytwo percent of the subjects received a higher education. The age
at the first drinking was 21.3¡¾6.5(mean¡¾S.D.) years, and the amount of alcohol drinking was 2.3¡¾1.0 (mean¡¾S.D.) bottles
of soju daily. The number of admission was 1.9¡¾1.9 (mean¡¾S.D.). 2) Among ¡®poor insight¡¯ group, false-negative rate on
NAST was 39.4% before treatment, and 33.3% after treatment. False-negative rate on NAST was significantly higher in the ¡®poor insight¡¯ group than ¡®partial¡¯ or ¡®good¡¯ insight group before (p=0.001) and after (p<0.001) treatment.
Conclusion£ºThese results suggest that alcohol-dependent patients with poor insight may report false-negative results on an alcoholism screening test like NAST. Therefore, during these the screening test, thorough evaluation on the subject's level of insight is required.
